What Are UKMLA Questions?

UKMLA questions are Single Best Answer (SBA) clinical vignettes used in the UK Medical Licensing Assessment Applied Knowledge Test (UKMLA AKT). Each question presents a patient scenario — including history, examination findings, and investigation results — and asks the candidate to select the single best answer from five options.

The UKMLA AKT contains 200 SBA questions across a 3-hour and 10-minute exam. Questions are drawn from all conditions listed in the GMC's UKMLA Content Map, which covers 430 conditions across medicine, surgery, paediatrics, obstetrics and gynaecology, psychiatry, and more.

Our UKMLA question bank is designed to replicate the style, difficulty, and clinical reasoning required by the real exam — giving you the most realistic and effective preparation possible.

What Makes Our UKMLA Questions Different

Not all UKMLA question banks are created equal. Here is what sets ours apart.

Mapped to the GMC Content Map

Every UKMLA question is tagged to a specific condition and patient presentation from the official GMC UKMLA Content Map. You always know exactly what you are revising.

Authentic SBA Format

Our questions mirror the exact format of real UKMLA exam questions — clinical vignettes with five plausible options requiring genuine clinical reasoning, not rote memorisation.

Full Explanations with NICE Guidelines

Every answer comes with a detailed explanation covering the diagnosis, investigations, management, and relevant NICE or BNF guidelines — so every question is a learning opportunity.

Progress Tracking by Condition

See exactly which UKMLA conditions you have practised and which you have not. Our coverage tracker maps your progress across all 430 conditions so no gaps remain.

Spaced Repetition Algorithm

Difficult questions resurface at optimal intervals based on your performance, using spaced repetition to maximise long-term retention of clinical knowledge.

Example UKMLA Question

Here is an example of the type of UKMLA question you will practise on our platform:

Cardiology · UKMLA Condition: Atrial Fibrillation

A 72-year-old woman presents to the emergency department with a 6-hour history of palpitations and mild breathlessness. Her ECG shows an irregularly irregular rhythm with no discernible P waves and a ventricular rate of 128 bpm. Her blood pressure is 118/76 mmHg and she has no signs of heart failure. She has a history of hypertension and type 2 diabetes. What is the most appropriate initial management?

A.DC cardioversion
B.Intravenous amiodarone
C.Rate control with bisoprolol and anticoagulation with apixaban
D.Rhythm control with flecainide
E.Digoxin monotherapy
Explanation: This patient has new-onset AF lasting >6 hours, so cardioversion carries a risk of thromboembolism. Rate control (bisoprolol) reduces symptoms, and anticoagulation (apixaban) is indicated given her CHA₂DS₂-VASc score of ≥2 (age, female, hypertension, diabetes). NICE NG196 recommends rate control as first-line for most patients with AF.

UKMLA Questions — FAQ

How many UKMLA questions are in the real exam?

The UKMLA Applied Knowledge Test (AKT) contains 200 Single Best Answer (SBA) questions. The exam lasts 3 hours and 10 minutes, giving approximately 57 seconds per question.

Are UKMLA questions the same as PLAB 1 questions?

The format is identical — both use Single Best Answer (SBA) clinical vignettes. The content is also very similar, as both exams are based on the GMC UKMLA Content Map. Our question bank is therefore equally effective for PLAB 1 preparation.

How difficult are UKMLA questions?

UKMLA questions are pitched at the level of a newly qualified Foundation Year 1 doctor. They test clinical reasoning and application of knowledge rather than pure recall. Questions require you to interpret clinical scenarios and select the most appropriate management — not just recall facts.

How many UKMLA questions should I practise?

Most successful candidates complete at least 1,000–2,000 practice questions before the exam. Our full question bank of 1,178+ questions, combined with multiple mock exams, provides comprehensive preparation.

Can I access free UKMLA questions?

Yes — our platform includes free UKMLA questions that you can access immediately after signing up, with no credit card required. A subscription unlocks the full question bank of 1,178+ questions.
